Request an Appointment

We know it's a big step to request an appointment for the first time. If you're a prospective new patient, please fill out the information on the following "New Patient Appointment" page and we will get back to you via email within 24 business hours

Things to Note about New Patient Appointments: 

-If you are requesting an appointment for ADHD (or will need treatment using a controlled medication), we require that the first appointment be in-person. This is in accordance with new DEA prescribing guidelines

-If you are wanting to continue past ADHD treatment, please have your records sent to our practice in advance.  You can contact your previous prescriber and have them fax the records to 919-214-5388 or email them to info@bravomentalwellness.com. If this information is not received in advance of your appointment, you may need to be fully re-assessed (including CNS-VS testing) prior to your provider issuing additional controlled medications 

-New patient appointments are scheduled for 90 minutes. We do a thorough evaluation and find that we typically use this entire amount of time
